Toyota 2.0 / 2.2 D-4D (1AD-FTV, 2AD-FTV, 2AD-FHV)Injectors
Injector wear and failure on the 2.0/2.2 D-4D
Why this rating?
Medium severity: It can leave you stranded or get worse if ignored, without destroying the engine.
Frequency common: The injectors appear among the most cited faults on all the reliability pages consulted for this engine, with several dozen cases on the Avensis T25 and Corolla Verso and several more on the Auris and RAV4, across all three displacements of the family.[1][2][3][4][5][6][7]
Symptoms
- The warning light comes on when the ECU detects an injector that is spraying badly or dripping.[2]Engine warning light: other faults that cause it · Garage soon
- The engine jerks or runs unevenly at idle when one or more injectors fail.[4]Jerking: other faults that cause it · Garage soon
- An injector that drips or sprays badly can cause black smoke on acceleration because of excess fuel that is not fully burned.[1]Black smoke: other faults that cause it · Garage soon
Why it happens
The common-rail injectors of this family lose spray precision with normal wear and with the use of low-quality or contaminated diesel, which results in injection faults documented repeatedly across several models and generations.[1][2][6][7]
If it is not repaired: An injector that drips unburned fuel can dilute the engine oil and damage the catalytic converter or the particulate filter if it is not corrected.[2]
Affected cars and years
Documented in all three variants of the family, with cases in the Avensis, RAV4, Auris, Corolla Verso and Verso.[1][2][4]

How much it costs to repair
Includes: Diagnosis and replacement of one or more common-rail injectors, with their coding in the ECU.[2][3]
Several owners cite around €800 for a single injector and between €1,400 and €1,900 when several are replaced; there are no official Spanish rates and it is not specified whether the repair was done at an independent workshop or at the dealer.
Prices in Spain, including VAT. A workshop hour costs on average €38–66 at independent garages and €77–134 at dealerships in Spain, depending on the region and the brand. How we calculate costs
How to check for it before buying a used car
- Ask for a cold-start test and accelerate gently, paying attention to jerking or black smoke; these are signs of worn injectors.
Parts needed
The part or kit that fixes this fault, according to the sources.
- Buy part at Autodoc (affiliate link, opens in a new tab)
Common-rail diesel injector
Sprays the diesel into the combustion chamber; it is replaced individually or as a complete set when it loses precision.

How to prevent it
- Refuelling at reputable filling stations and not running the tank down to reserve helps stop the injection system from drawing in impurities or water from the bottom of the tank.
Recalls, campaigns and claims
As of 27 September 2026, we have no record of recalls, campaigns or extended manufacturer warranties for this fault. Check with an official dealer using the chassis (VIN) number: they will tell you whether the car has any outstanding campaign.
What owners say
Frequently asked questions
Do all four injectors have to be replaced at the same time?
Not necessarily: several owners cited by fiches-auto.fr only replaced the injector that had failed, although some workshops recommend checking them all if the car has high mileage.[4]
Does fuel quality matter?
The sources consulted point mainly to normal wear and to the sensitivity of the common-rail system, although low-quality diesel is a known wear factor for this type of injector.[1]
Can a failing injector damage the engine?
It can cause jerking and loss of power and, if it drips unburned fuel into the oil, contribute to engine oil dilution.[2]
What can you do now?

Other 2.0/2.2 D-4D faults
- Critical severityFrequency: Very frequentCylinder head and blockWarped cylinder head gasket and oil consumption on the 2.2 D-4DYes: the 2.2-litre block (2AD-FTV and 2AD-FHV D-CAT) has numerous documented cases of warped cylinder head gaskets and oil consumption or dilution in the Avensis, RAV4, Auris, Corolla Verso and Verso, with repairs that sometimes require replacing the engine.Appears at around 40,000 km
- Medium severityFrequency: Very frequentEmissions controlEGR valve clogged with sootYes: the EGR valve clogs with soot very frequently across the whole 1AD/2AD D-4D family, especially with urban use and short journeys, and it usually causes a loss of power or the engine warning light to come on.Appears at around 45,000 km
- Medium severityFrequency: CommonEmissions controlD-CAT particulate filter cloggingYes: the D-CAT particulate filter, and that of later versions without the badge, becomes saturated with soot if the car does not cover enough open-road miles, with the DPF warning light on and forced regenerations costing around €180 at a workshop.
More faults with the injectors on other engines and cars
- High severityFrequency: Very frequentOM651Delphi piezo injector failure (220/250 CDI)Yes: 220 and 250 CDI models built between 2008 and 2011 had Delphi piezo injectors that were prone to failing and seizing in the cylinder head. Mercedes-Benz replaced them under warranty in 2009 and switched the 220 CDI to solenoid injectors from 2010-2011, but the 250 CDI kept them.Repair (garage): €3,600–€5,900
- High severityFrequency: CommonBMW N52 / N53N53 piezoelectric injector failureThe N53's piezoelectric direct injectors fail frequently and cause misfires, rough idle and limp mode; replacing them is expensive and they must be coded.
- High severityFrequency: CommonMZR-CDOil dilution from leaking injector seals and DPF regenerationsOn the 2.0 and 2.2 MZR-CD, injector seals that age badly and incomplete particulate filter regenerations on short journeys can let unburnt diesel into the sump, diluting the oil and leaving the engine with less lubrication.
- High severityFrequency: CommonOM642Wear and failure of the Bosch piezo injectorsThe OM642's Bosch piezo injectors can fail from around 150,000-200,000 km, especially with poor-quality diesel; they cannot be repaired, only replaced. Because it is a V engine, those on the rear bank are harder to reach, which pushes up labour costs.Repair: €4,500–€6,500
- High severityFrequency: Common2.2 CDI (OM646)Injector seal leak with carbonisation around the seatYes: the copper washer that seals each injector can harden and let combustion gases through, forming a black crust of carbon around it. It is a known fault on Mercedes CDI diesels of this era, with injectors described as fragile on both the OM611 and the OM646.
- High severityFrequency: Common2.2/2.5 dCiInjection pump or injector failureThe 2.2 dCi's common rail injectors fail fairly often on the X-Trail and Almera, with cases of seizing and leaks; on the Almera with the older YD22DDT, the documented fault is in the injection pump electronics, not the injectors.
